Join ACI's Clinical Network
Interested in joining ACI Specialty Benefits' network of clinicians? There are two steps to apply. All documents may be downloaded and submitted below.
Step 1:
Download and complete the Provider Packet (Provider Survey, Independent Contractor Agreement and W-9) and Program Utilization Form.
To become a center provider for ACI, please contact Provider Relations at 858.736.0047 or email us at provider-relations@acispecialtybenefits.com.
Step 2:
Upload these documents and a copy of your current malpractice insurance and license.
NOTE: All documents must be submitted in order for verification and addition to panel. Please maintain a copy of these documents for your records. Upon receiving your information, it will be entered in ACI’s EAP database and you may receive referrals from ACI. If you are unable to upload the required documents electronically, please email completed documents to provider-relations@acispecialtybenefits.com or fax to 858.452.7819 Attn: EAP Provider Relations.

Interested in joining ACI Specialty Benefits' network of crisis facilitators? There are two steps to apply. All documents may be downloaded and submitted below.

Step 2:

About ACI’s Training Programs:
ACI training offerings are generally one hour Live ‘n Learn seminars: about 45 minutes for the presentation and 10-15 minutes for a Q&A session. These sessions are scheduled during times that are convenient for our clients.
ACI offers topics under the categories Management, Stress, Communication and Health and Wellness. Some examples of the training titles are Effectively Communicating in the Workplace, Managing Organizational Change and Team Building.
Training professionals, trainers by trade, and members of ATD (Association for Talent Development fka the American Society for Training & Development) and SHRM (Society for Human Resource Management) are encouraged to apply.
